Summer 2010

• Registration Period is on April 5-9
• Classes Begin on April 12
• Classes End on May 22

First Semester 2010-2011

Enrollment Dates:

• Freshmen (Regular) on May 3
• Second and Upper year Levels including First Year Irregulars on May 30
• Internship Training for MT, PT and OT, please refer to your respective Deans

Classes Begin on June 15, 2010

Requirements for Enrollment

New Students:

• High School Graduates:

1.  Form 138-A (4th Year High School Report Card)
2. NSO Birth Certificate (Original)
3. Certificate of Good Moral Character duly signed by the school authority

*Qualifying Examination is required for freshmen seeking admission to the College of Nursing, College of Occupational Therapy and College of Physical Therapy. Please see the Office of the Dean for details.

• College Transferees:

1. Certificate of Transfer Credential
2. Informative copy of Credits earned with grades
3. NSO Birth Certificate (Original)
4. Certificate of Good Moral Character duly signed by the school authority

• Old Students:

1. School I.D.
2. Latest Final Admission Slip with Grades
